Please arrive 15 minutes early to complete any necessary paperwork, or save time by filling out our new patient forms online ahead of your visit.

Bring a photo ID and your dental insurance card, if you have one.

We accept most major PPO dental insurance plans. If you do not have insurance, ask us about our membership plan.

Let us know about any dental anxiety or special needs ahead of time. We offer sedation options and Hypno Dentistry to help you feel comfortable.
