No, and that is the point. There is no insurance company involved, so there are no claim forms, no waiting periods, and no annual maximum that cuts you off partway through the year. You pay us directly and the plan is between you and this practice.
$32 per month or $385 per year for one person. $83 per month or $1,000 per year for a family of up to four. Additional family members beyond four are $25 per month or $300 per year.
Yes. There are no waiting periods, so your covered care is available as soon as you join. That is a real difference from most dental insurance, which often makes you wait months before major treatment is covered.
You get 15% off basic and preventative care and diagnostics, and 10% off major treatment such as crowns, bridges, and implants. There is no annual maximum, so the discount applies no matter how much care you need in a year.
The discounts apply to treatment across the practice, and every specialty works in this office. That includes periodontics with Dr. Farshid Ariz and endodontics with Dr. Ali Vaziri, so you are not sent elsewhere and charged separately.
We do not participate in Medicaid, Medicare, or other government-funded insurance programs. If you are on one of those, call us and we will talk through your options, including this plan.
